Serving At Calvary

We’re grateful for your desire to serve! At Calvary Maricopa,

before you begin the application process, we ask that those stepping into ministry have:

1. been regularly attending for at least six months.
2. a personal relationship with Jesus Christ, and are in agreement with our doctrinal beliefs.

The standard serving application takes about 10 minutes to complete. If you are applying to teach God’s Word, there is an additional section that extends the process to approximately 20–30 minutes.

For those serving in ministries involving vulnerable populations—such as Calvary Kids, Relentless Youth, or our Safety Team—a background check is required ($10).

Introduction

Dear Brother or Sister in Christ,

Thank you for your decision to serve the Lord here at Calvary Maricopa.

Your desire to serve is a natural and proper response to the work of Christ within you. Where you serve is a big question to answer. In the following pages you answer a series of questions that are designed to help us get to know you better and to help you determine your place in the Body of Christ. We all have a unique blend of talents, traits, gifts, and past experiences that God will use to accomplish His will.

The Apostle Paul uses the ‘body’ as a picture of how the local church should function. Each one of us is a unique part of that body, and just like our human bodies, each part serves a specific purpose. We want to avoid you serving in an area of ministry for which you were not designed. Taking your time to fill out the Ministry Questionnaire sincerely will help reduce the chances of placing you in the wrong area and will maximize the potential for you to experience one of the greatest joys you’ll ever know – being used by God in the capacity for which you were designed.

Ephesians 2 tells us that we are God’s workmanship; created in Christ Jesus for good works, which He planned for us before the foundations of the earth. Enjoy the excitement and challenge of this truth as you continue through the following Ministry Questionnaire and discover God’s life for your will. Then, have a wonderful, fulfilling time serving Him with all your heart.

Servant Requirements

  • Must be saved
  • Must have attended Calvary Maricopa for at least six months
  • Must have completed this ministry application
  • Must be actively involved in this ministry by regular attendance

Application Process Steps

  1. Please read and review our 'Statement of Faith' located here.
    • If there is anything you do not agree with, before you complete the application, please make an appointment to speak with the ministry leader or a pastor to discuss those disagreements. (Amos 3:3)
    • If you agree with the Statement of Faith, please continue with the application.
  2. If applying to serve in safety, Calvary Kids, or Relentless Youth, please fill out the Background Check Authorization at the end of this form.


Before continuing with the application process, please make an appointment to speak with the ministry leader or a pastor to discuss your disagreements with the Statement of Faith. We would love to hear what you think.

Background Check Authorization

Consent and Waiver / Background Information Form

In consideration of my role with Calvary Maricopa, Inc., its affiliates, integrated auxiliaries and supporting organizations, hereinafter “Calvary Chapel Maricopa” and recognizing the importance of my character being above reproach, I HEREBY CONSENT TO THE FOLLOWING:

  1. Complete in full a Calvary Chapel Maricopa Consent and Waiver / Background Information Form
  2. Allowing Calvary Chapel Maricopa to screen me through the Arizona Department of Law Enforcement or any other state, county, municipal agency or private firm for the purpose of accessing and reviewing Arizona and national criminal history records as well as any other historical or background records pertaining to me.
  3. Allowing Calvary Chapel Maricopa to verify the information and to contact any personal references listed on the Background Information Form.
  4. Allowing Calvary Chapel Maricopa to verify current and prior employment.
  5. Provide Calvary Chapel Maricopa with a photocopy of my current state driver’s license, official state identification card or valid passport.
  6. Make myself available for a personal interview by a Pastor or overseer of Calvary Chapel Maricopa.

I HEREBY WAIVE, RELEASE AND HOLD HARMLESS FROM LIABILITY all persons, organizations, and other entities which provide references or information to Calvary Chapel Maricopa pertaining to me or my background.

I HEREBY FURTHERWAIVE, RELEASE AND HOLD HARMLESS FROM LIABILITY Calvary Chapel Maricopa, it’s staff, employees, volunteers, and agents with regard to any decision that it makes on my application for involvement with Calvary Chapel Maricopa based on the information I provide or that is obtained through the criminal history and background screening process.

I CONSENT to a copy of this Consent and Waiver Form together with the Background Information Form being furnished to any reference that I have provided to Calvary Chapel Maricopa and to any other person, organization or entity that Calvary Chapel Maricopa deems necessary in connection with its investigation of my background, character or qualifications.
